Age-Friendly Care: Applying the 5Ms of Geriatrics to Post Acute and Long Term Rehabilitation
REGISTER NOWWith a rapidly growing aging population and rising health care costs in a fractured health system, a recent initiative called Age Friendly Care has taken root among impacted providers, beneficiary stakeholders, and government agencies. Beginning in 2025 the Centers for Medicare and Medicaid Services has added an Age Friendly Hospital Measure, and the FY 2025 SNF PPS proposed rule included a request for information about a resident “well-being” quality measure concept. Age-friendly care emphasizes a person-centered approach to maintaining the health of older adults based on evidence-based care that prioritizes the older adult’s specific health outcome goals and care preferences, regardless of setting, in order to improve health outcomes and prevent avoidable harm.
This 4-hour symposium, intended for physical and occupational therapy, and speech-language pathology professionals, post-acute and long-term care nurses, and nursing home and assisted living administrators, will include presentations and interactive panel discussions of the team approach to incorporate the 5Ms of geriatrics: Mind, Mobility, Medications, Multi-Complexity, and Matters Most into everyday resident care.
