With a rapidly growing aging population and rising health care costs in a fractured health system, a recent initiative called Age Friendly Care has taken root among some impacted providers, beneficiary stakeholders, and government agencies. Age-friendly care provides older adults with personalized, evidence-based care focused on their goals and preferences, regardless of the setting in which they receive that care. Implementing this approach improves health outcomes and prevents avoidable harm.

This 4-hour workshop-style symposium, intended for physical and occupational therapy, and speech-language pathology professionals, post-acute and long-term care nurses, and nursing home and assisted living administrators, will include presentations interactive panel discussions, and roundtable discussions of the team approach to incorporate person centered care using one example age friendly care framework, the 5Ms of geriatrics: Mind, Mobility, Medications, Multi-Complexity, and Matters Most into everyday resident care.

Learning Objectives:

  • Describe the 5Ms of geriatrics principles, their relationship with person-centered care, and their potential application across rehabilitation disciplines and healthcare team.
  • Implement components of age-friendly principles into the culture of care in post-acute care settings across disciplines.
  • Develop an action plan with 3 strategies for creating stronger person-centered care practices in your facility/agency/clinic.
